Congregate Housing Services Program Coordinator – Part Time 25 hours per week

Job Summary:

Responsible for the State of New Jersey subsidized Congregate Program which includes coordinating the service delivery of meals, housekeeping and personal assistance for eligible residents of an affordable housing facility. The CHSP coordinator is required to adhere to all State policies, rules and regulations in the identification and the provision of support and encouragement to those eligible residents in need of program services.

Job Duties:

  • Receive requests for service and coordinates interviews of potential participants and explains the available program options to develop an individual service plan
  • Prepare yearly budget, obtain all related documents for the budget and inputs into SAGE software. Track progress to ensure compliance.
  • Establish and maintain a waiting list and written procedures for residents who are eligible for CHSP if there are not adequate funds or staffing to serve all
  • Ongoing assessment of participants eligibility needs and financial status
  • Hiring, training and supervision of the program staff and volunteers
  • Order all food and consumables to ensure necessary inventory is maintained
  • Schedule, provide and track all meals, housekeeping and personal care per the program guidelines
  • Deliver meals to participants on an as needed basis
  • Prepare paperwork and forward to appropriate agency for payment processing
  • Prepare monthly reports and requests for payment in a timely manner and submit to appropriate agency
  • Enter program invoices for payment into Property Management software and forward to Accounting for payment processing
  • Prepare and deliver monthly invoices to the residents on the program
  • Collect and process payments from Residents. Record all payments per the program guidelines
  • Maintain a strictly Kosher kitchen and dining facility
  • Establish contacts with healthcare and service agencies and the maintenance of a current resource file, especially the appropriate agencies that service the senior population
  • Regularly attend the training programs sponsored by the Division of Aging Services
  • Work with the Activities Coordinator regarding meals and programs that are held in the dining space
  • Assist residents in applying for assistance programs including but not limited to PAAD, LIHEAP, etc.
  • Coordinate blood work requests with designated vendor
  • General office duties
  • Perform other related duties directed by supervisor in a cooperative manner.

Job Requirements

  • 4 years of experience in providing direct services to elderly persons such as social services, recreation, health care, education and\or training, counseling, nutrition, housekeeping and\or personal care or senior housing
  • An Associate’s degree with a major course of study in gerontology or a related field of study in providing direct services to the elderly or 4 years of related experience

  • Maintains a positive behavior and attitude while providing quality services to residents and/or family members.
  • Works at maintaining a good rapport and cooperative working relationship with departments, staff, co-workers and management.
  • Responds timely to phone calls, emails, etc. and/or requests for information.
  • Represents JFH Management in a positive and professional manner at all times.
  • Maintains and obeys the dietary laws for maintaining a Kosher environment in our community.
  • Accepts assignments in a cooperative and positive manner.
  • Takes ownership to maintain a work environment that is free of debris as well as keeps general work area neat, clean and orderly.
  • Performs all duties of job to assure resident and personal safety and the protection of co-workers and facility property.
  • Maintains professional growth and development through seminars, workshops and professional affiliations, if applicable.
